simply print it, fill it out, include your $500.00 deposit and mail
it to us today.
If you are unable to print the form for any
reason, you may write all the information requested on the form on
any piece of paper and mail it with your deposit.

REGISTRATION: If you wish to pay your deposit by credit card
online, you can complete the following short form for registration:
PRE-REGISTRATION:
You must pre-register by sending a non-refundable deposit of $500.00
for the course only. $1,500 for course and Full service patient
package (personal checks accepted 30 days piror only). Final
payment is due the first day of class, or 30 days before your
testing date, whichever comes first in the form of cash,
travelers cheques, money orders, cashiers check. No personal checks
will be accepted at the course.
NOTE: If you
pre-register for a course and then do not get the test date you
requested from the board, your WDC deposit will be applicable to a
future course for up to 1 year.
